As Christians, we are taught to evangelize, that is, to share the Good News with other people we encounter.

We’re told it’s our responsibility.

For me, it’s not something I have an easy time doing. In an increasingly secular society, where “Jesus Freaks” are not admired as much as they are ridiculed, and where we can get into legal trouble if we don’t properly separate Church and State, I don’t want to risk alienating others by sharing with them my religious beliefs. But I – shamefully – know that the more honest statement is: I don’t want to risk alienating myself with others.

若不及時處理糖尿病有以下的急性併發症

短期性併發症

 

1. 高血糖 - 糖尿病昏迷酮酸中毒

    病徵及症狀

  • 起因 –  逐漸發生
  • 皮膚 –  紅潤及乾燥
  • 表現 –  昏昏欲睡, 若果不立刻醫治, 昏迷可致死亡
  • 口氣 –  水果氣味
  • 作悶及嘔吐 – 有
  • 舌頭 – 乾涸
  • 饑餓 – 沒有
  • 口渴 – 有
  • 痛症 – 腹痛
  • 呼吸 – 吃力並急速
  • 血糖度數很高, 高過300mg/dl 或以上

   高血糖的處理

  • 若血糖高過300mg/dl, 及有以上高血糖症狀應立刻致電您的醫生要求指示。
  • 若您忘記服食糖尿藥或注射胰島素, 應立刻服藥或注射胰島素。
  • 若發現糖尿病人昏睡或昏迷, 須要立刻召電911求助。

2.低血糖 - 胰島素反應 

   病徵及症狀

  • 起因 –  突然
  • 皮膚 –  蒼白及出冷汗
  • 口氣 –  正常
  • 作悶及嘔吐 – 沒有
  • 舌頭 – 濕潤
  • 饑餓 – 有
  • 口渴 – 沒有
  • 痛症 – 頭痛
  • 呼吸 – 正常至到急速/微
  • 血糖度數很低, 低過70mg/dl 或以下
  • 有些人或有血糖70-110mg/dl 都會感覺有低血糖反應。

   低血糖的處理

  • 若檢到有血糖低過70mg/dl, 及有一或兩個以上低血糖症狀, 若病人還可講話及行走, 應立刻吃食物,或飲四安士果汁,或服食三粒糖丸同一杯清水一起飲。
  • 但是若病人是昏睡便不可硬要她/他飲食, 這是很危險的。若病人昏迷, 應立刻召電911, 這是急症, 應送病人到急症室求緊急處理。

 

糖尿病人經突發低/高血糖的處理後,可以很快便回復正常的運作。

 

 

Acute Complications of Diabetes if not treated promptly

Short term complications

 

1. Hyperglycemia – Diabetic Ketoacidosis

    Signs and symptoms

  • Cause –  slow onset
  • Skin –  flush and dehydrated
  • Presentation –  drowsy and sleepy, unconscious can lead to death if not treated immediately
  • Breath –  fruity smell
  • Nausea and vomiting – Yes
  • Tongue – dry
  • Hunger – no
  • Thirst – Yes
  • Pain – abdominal pain
  • Respiration – deep and fast
  • Blood glucose level very high, over 300mg/dl or higher

   Treatment of Hyperglycemia

  • If blood sugar has gone up to 300mg/dl and has above symptoms, should call your doctor immediately for advice.
  • If you forget to take your diabetic medicine, either take your pills or injection of insulin right away.
  • If find diabetic patient drowsy or unconscious, should call 911 right away for help.

2. Hypoglycemia – Insulin reaction 

    Signs and symptoms

  • Cause –  sudden
  • Skin –  pale and diaphoretic
  • Breath –  normal
  • Nausea and vomiting – No
  • Tongue – moist
  • Hunger – Yes
  • Thirst – Yes
  • Pain – headache
  • Respiration – normal to rapid/shallow 
  • Blood glucose level very low, below 70mg/dl or lower.
  • Some people may have blood glucose 70-110mg/dl and will still experience low blood sugar reaction

   Treatment of hypoglycemia

  • If tested and has blood sugar lower than 70mg/dl and has one or two of the above hypoglycemic symptoms, if the patient still can talk and walk, eat some food right away, or drink 4 ozs of fruit juice, or take 3 sugar tablets with a glass of water.
  • But if the patient is drowsy, do not force the patient to drink or eat, this is very dangerous. If the patient is unconscious, call 911 immediately, this is medical emergency, and the patient should be taken to Emergency Room for urgent treatment.

 

After treatment of acute hyper/hypoglycemic treatment the diabetic patient will quickly resume his/her usual activity.

如何處理背痛

許多疾病及其他的情况都會引致背痛。 導致背痛的原因可能是背脊骨有問題, 或身體其他部位有問題而引致背痛 。很多時醫生都找不到背痛的起因。

常見的起因

  • 背肌肉受損傷, 因扭傷或拉傷; 因肥胖而背肌肉受到長期性的負苛; 有時因舉重或抬重東西或懷孕後期使到背肌肉有短暫的不沈常的負苛而使到有背痛。 
  • 脊骨的疾病及損傷, 包括因意外或因骨質疏鬆症影響骨折或骨裂。
  • 因年長, 受傷及有遺傳傾向的疾病而使到有脊骨退化症, 及風濕影響到有背痛。 
  • 因脊骨神經線疾病, 如椎間盤突出或脊椎骨狹窄, 壓着神經線而使到有背痛。
  • 急性/慢性腎石症及腎炎都可引致背痛。

少見的起因

  • 脊骨風濕發炎症如强直性脊骨炎及有關的情況。 
  • 脊骨發炎會在椎間盤發炎;骨髓炎; 脊骨有膿腫;肚腹, 盆骨發炎; 或血內有 菌都可引至背痛. 這些都是緊急情況, 應立刻求醫。   

症狀

背痛的症狀有很多樣化,包括發燒, 最近創傷, 背部受損, 減磅, 因有癌症歷史而擴散到脊骨, 腦神経線症狀, 如痲痺, 手腳軟弱, 或更嚴重的情況會使到失禁 。 由於背痛會引發其他不適的症狀,可幫助知道背痛的起因, 例如:-

1. 背脊扭傷或拉傷

  • 因抬重, 用力過度去推或拉重。
  • 背肌肉, 屁股, 大腿很多時都感覺疼痛及僵硬。背肌肉輕輕觸摸也會很痛 。

2. 脊骨退化性關節炎

  • 長時間的勞損而引致的關節炎會導致背痛僵硬, 及不可能彎腰。

3.  椎間盤突出

  • 椎間盤破裂或向外脫出,會壓着脊骨神經線, 導致有劇烈的背痛, 彎腰或扭腰會更痛, 痛可散至到整隻脚。

4. 骨質疏鬆症

  • 脊骨會疏鬆及弱, 很容易折烈-----普遍發生在收經後的女士們, 年老的女人及男人 。 若胸椎下半及腰椎有壓縮骨折, 除了有背痛還不可彎腰。  除非有骨折斷, 否則骨質疏鬆症是不會引致背痛, 及有任何痛楚。

5. 脊骨及附近的內臟有癌症

  • 背部會時常感到痛楚, 及躺平會更痛.  雙腳痲痺, 軟弱及有針剌感覺,疼痛還會持續的加劇。若癌症擴散到脊骨神經線,或會影響大小便失禁。

6. 纖維肌痛綜合症

  • 除了有背痛, 身, 頸部, 肩膊, 膝頭, 手肘都會感到全身痛, 及早上感覺特別僵硬。早上睡醒,通常會感到有異常的疲倦。   就算一夜睡得好都感覺到很疲倦;  及有特別位置輕輕觸摸都很痛。

7. 關節發炎

  • 如强直性脊柱炎及有關的情況 。常有背痛,  盤骨部, 頸, 胸及雙腳也會  有痛及僵硬, 及非常疲倦。

診斷

  1. 病歷及症狀描述。
  2. 身體檢查, 醫生會檢查您的背肌肉及脊骨, 他須要您做一些動作, 檢查您有没有痛, 有沒有肌肉觸摸痛及軟弱, 僵硬, 痲痺, 及反應異常。
  3. 檢查後,您的醫生才會決定您的背痛是否嚴重及痛的因素是肌肉扭傷, 或因肥胖所引起,  懷孕或任何不緊急的因素 。您便不須 要任何試驗。 但是若您的醫生懷疑您有更嚴重的脊骨或脊骨神經線問題, 或有急性損傷, 例如車禍,跌倒, 或任何意外, 或您的背痛持續多過12個星期, 用了所有家庭的藥物及治療都沒有進展, 您或須要進行以下更進一步的檢查:
  • 背部X-光鏡
  • 驗血
  • 檢驗小便
  • 脊骨磁力共振MRI
  • 電腦掃描CT Scan
  • 神經線傳導測驗nerve conduction studies, 及肌肉電圖electro-myography 去診斷您的肌肉或/及神經線有沒有受損。
  • 骨掃描bone scan, 特別是您有癌症的病歷, 去診斷癌細胞是否擴散到脊骨。

 

預計背痛期限

視乎背痛的起因:

  1. 若因用力過度而扭傷----數天或數星期, 少過6-12星期及您可逐漸回復正常運作。  在這段時期, 您應避免攜重物件, 長時間坐着, 或突然間彎腰或扭腰, 直至到您的背痛有好轉及復元。
  2. 孕婦的背痛通常產後會有好轉。
  3. 肥胖的人或須要减體重才感背痛舒緩。
  4. 若因有腎石及腎發炎而使到有背痛, 服完一兩星期的特效消炎藥會消炎, 或腎石自然排出或須動手術才可取出腎石。 在一兩天或數天後背痛通常便會有好轉。
  5. 較嚴重的脊骨問題會影響到骨及神經線, 背痛或會持續數年或數月。

預防

  • 每日做增強背部的運動, 記着做背部運動之前及後要做伸展的舒緩運動。
  • 防止做任何引致背部損傷的活動。
  • 行走坐卧, 要保持良好姿勢。
  • 若側面睡, 可放小枕頭在兩膝頭中間, 若背睡, 可放枕頭在膝下。 
  • 練習仰卧起坐運動去堅強腹部肌肉,可幫助強化脊骨。經常步行或游水令腰骨會更強壯。
  • 學習舉起物件時先蹲下的動作, 然後用盤骨部及雙腳幫助抬重。避免彎腰或扭腰從地上拿起重物。
  • 防止長時間坐立, 及彎頸或背部。
  • 平時應穿著低過一吋半的軟底結實鞋。女士們應避免長時間穿著高跟鞋。
  • 防止有骨質疏鬆症---------每日從飲食或保充劑吸取足夠的鈣及維他命D, 不要吸煙, 飲少量酒, 每天做運動, 曬太陽。

治療

          很多時背痛都不是很嚴重的, 及可以用以下的方法去處理:-

  • 限卧床休息一至兩天。 
  • 有痛可服Acetaminophen (Tylenol 及其他牌子) 或任何消炎薬, 如Aspirin, Ibuprofen (Advil, Motrin 及其他同一功效的藥物), 或 Naproxen( Aleve) 可減痛及消炎。
  • 如有須要, 可短期服肌肉鬆弛劑或醫生寫的更強的治痛藥方。 
  • 用熱敷。
  • 按摩, 針炙, 指壓, 脊骨推拿或會有幫助, 要預先請教您的醫生才好去做以上治療。
  • 慢慢逐漸回復正常活動; 之後應防止抬重物件; 避免久坐及突然彎腰或扭身。每天去散步可堅固脊骨肌肉。
  • 慢性背痛(超過12個星期 ) 嘗試太極或瑜珈等運動。
  • 慢性背痛也可因抑鬱症等心理問題而影響,須 請教心理科醫生。

何時須要打電話向專業人士求助?

要致電您的醫生, 若有以下情況:-

  • 有劇烈的背痛而影響正常生活。
  • 跌倒, 有車禍或任何急性受傷 。
  • 幾日輕微背痛變嚴重或持續多過一兩星期。
  • 除了背痛還有其他症狀, 如突然減磅, 發燒, 發冷, 或小便時有灼熱感。
  • 突然感覺軟弱, 雙腳或雙手感覺痲痺及針刺。
  • 腹股溝或直腸感到麻木及有困難控制大小便。
  • 有腫瘤病歷而突然有持續的背痛而又與受傷無關。

預測

多過百份之九十有背痛的人都會經過内科治療而復元。只有小部份, 百份之五有背痛的人有其他的症狀維持多過12個星期, 很多時不很嚴重。 若慢性背痛沒有新或急性症狀, 而又可做日常運作, 而沒有很大痛楚, 許多人都可忍受的。

 結論

保持健康的脊骨須要時常有良好的姿勢,日常多做些強健肌肉的運動, 進食健康飲食去預防骨質疏鬆症, 不吸煙, 飲着量的酒, 保持健康體重,可幫助防止有背痛。 

 

 

Back Pain – how to deal with it

Back pain can be a symptom of many different illnesses and conditions.

The main cause of the pain can be a problem with the back itself or by a problem in another part of the body.  In many cases, doctors cannot find the cause for the pain.

Common causes

-Stress or injury involving back muscles, including back sprain or strain; chronic overload of back muscles caused by obesity; short term overload of back muscles caused by any unusual stress, such as lifting or pregnancy.

-Disease or injury involving the back bones (vertebrae), including fracture from an accident or as a result of the osteoporosis.

-Degenerative diseases and arthritis, a “wear and tear” process that maybe related to aging, injury and genetic disposition.

-Disease or injury involving the spinal nerves, including nerve injury caused by a protruding disk (fibrous cushion between vertebrae) or spinal stenosis (a narrowing of the spinal column).

-Kidney stones or a kidney infection (pyelonephritis).

Rare causes

-Inflammatory arthritis, including ankylosing spondylitis and related conditions.

-Infection, which may be in the disk space, bone (osteomyelitis), abdomen, pelvis or bloodstream, and abscess in the spine. These are more acute situations, and needs immediate medical attention.

Symptoms

Back pain symptoms varies, this may include fever, recent trauma, injury to back, weight loss, a history of cancer with metastasis to the spine, neurological symptoms such as numbness, weakness of arms or legs, or more severe condition can cause incontinence.

Back pain usually is accompanied by other symptoms that may help to pinpoint to its cause, for example:-

1. Back sprain or strain 

-After heavy exertion; lifting, pushing and pulling.

-Muscles in the back, buttocks and thighs are often sore and stiff. Back muscles maybe tender to touch.

2. Degenerative arthritis of the spine

-Besides back pain, there is stiffness and trouble bending over, which usually develops over many years.

3. Protruding disk (Herniated disk)

-The bulging or herniated disk compress on the spinal nerve causing severe back pain. Pain gets worse during bending and twisting, pain may spread down to one leg.

4. Osteoporosis

-The spinal bones got thinned and weakened and easily get fractured. Most commonly happened in postmenopausal women or elderly women. It can also happen in elderly men with osteoporosis. When there is compressed fracture of the mid and lower spine, people may have a crooked back and stooped over together with back pain.  Osteoporosis does not cause back pain, unless there is a bone fracture in the spine.

5. Cancer 

-Cancer in the spinal bones or nearby structures may cause back pain. It is consistent and may become worse when lying down. Numbness, weakness or tingling of the legs that continues to get worse. If the cancer spreads to the spinal nerves that control the bladder and bowels, there may be bowel or bladder incontinent.

6. Fibromyalgia

-Besides back pain, one will experience stiffness in the trunk, neck, shoulders, knees and elbows. Pain maybe generalized soreness, and stiffness is often worse in the morning. People usually complain of feeling abnormally tired even after a good night sleep, and there are specific areas that are painful to touch, called tender points.

7. Inflammatory arthritis

-It includes ankylosing spondylitis and related conditions. There is pain in the lower back, with morning stiffness in the back, hips or both. Also can have pain and stiffness in the neck or chest and feeling extreme tired.                                               

Diagnosis

1. Medical history and description of symptoms.

2. Physical exam-your doctor check on your back muscle and spine, move you in certain ways to check for pain, muscle tenderness, or weakness, stiffness, numbness or any abnormal reflexes.

3. Your doctor will determine your back pain is serious or not, or the causes are by muscle strain, obesity, pregnant or any other causes are not urgent, then you may not need any tests.  However if your doctor suspects serious problem involving your vertebrae or spinal nerves or you have an acute injury e.g. car accidents, falls, or any accidents, and if your back pain lasted longer than 12 weeks with no improvements after initial home remedy treatment, you may need one or more of the following tests:-

1. X-rays of your spine.

2. Blood tests.

3. Urine test.

4. Spinal magnetic resonance imaging (MRI).

5. Computed tomography (CT) scan.

6. Nerve conduction studies and electromyography to determine   whether nerves, muscle or both maybe injured.

7. Bone scan, especially if you have previous history of cancer, to rule out metastasis to the spine.

Expected Duration

It depends on the causes:-

-Strain from over exertion, days or weeks, less than 6-12 weeks and you may be able to return to gradual normal activities. During this time, you should avoid heavy lifting, prolonged sitting or sudden bending or twisting until your back pain gets better or resolved.

-Pregnant women’s back pain almost always will get better after delivery.

-Obese individuals may need to lose weight before back pain eases.

-When pyelonephritis or kidney stones causing back pain, once starting antibiotics for one to two weeks for kidney infection and when kidney stones passed out of the body, medically or surgically, back pain will resolve in one or two days.

-More serious spinal vertebral problems affecting the bones and nerves, back pain may last for months or years.

Prevention

-Do back strengthening exercise daily- but remember to do stretching before and after.

-Prevent any activities that may injure your back.

-Maintain a good posture, sitting and standing at all times.

-Sleeping on your side, with a pillow between knees, or lying flat on back with a pillow under your knees if you can, to relax your back while sleeping.                                 

-Practicing abdominal crunches to strengthen abdominal muscles, which support your lower back.  Walk or swim regularly to strengthen your lower back.

- Learn to lift objects from a squatting position, using your hips and your legs to do heavy work.  Avoid bending and twisting your back to lift any objects from the floor.

-Avoid sitting or standing and bending your neck and back for extended period of time.

- Wear soft sole shoes with heels less than 1 and one-half inches high. Women avoid wearing high heels for extended time.

- Prevent osteoporosis by getting enough calcium, Vit.D, avoid smoking and moderate consumption of alcohol, and do regular exercises and get enough sunshine.

Treatment

Most episodes of back pain are not serious and may be treated with:-

-Limited bed rest (no more than 2 days)

-Acetaminophen (Tylenol and others) for pain or any oral anti-inflammatory drugs such as aspirin, ibuprofen (Advil, Motrin and others)  or Naproxen(Aleve, Naprosyn), for pain and inflammation.

-Muscle relaxants or prescription pain relievers (may be more stronger ) if necessary for a short period of time.

- Hot compresses, massage therapy, acupuncture, chiropractor’s treatment, and acupressure may help.

-Return to normal activities gradually and avoid heavy lifting, prolonged sitting or sudden bending and twisting temporarily.

-Chronic back pain(more than 12 weeks) can try practising Tai Chi or Yoga exercise.

-Chronic back pain maybe due to psychological problem, like depression-may need to consult psychiatrist for assessment.

-If your back pain is related to more serious disorders of the vertebrae or spinal nerves, or it has not improved over a few weeks, you may need to consult a neurologist, orthopedic surgeon, rheumatologist, endocrinologist, and pain specialist, and if you have cancer history, refer to the oncologist.

When to call a professional?

Contact your doctor if:-

-Severe back pain hinders you to perform normal daily activities.

-You have a fall, auto accident or acute trauma of any kind to your back.

-Mild back pain gets worse after a few days or persists more than a week or two.

-Back pain is accompanied by weight loss, fever, chills or urinary symptoms, like burning on urination.

-You develop sudden weakness, numbness or tingling in your upper or lower extremities.

-You develop numbness in the groin or rectum or difficulty controlling bladder or bowel functions.

-You have had cancer history and you develop persistent back pain not related to any acute injury or trauma.

Prognosis

More than 90% of people with back pain get better after conservative medical treatment. Only 5% of people with back pain will have symptoms for more than 12 weeks and for most of these people the cause is not serious. As long as chronic back pain has no new or acute symptoms, and one can perform daily activities with no distress, many can live with it. 

Conclusion

Maintaining a healthy spine needs constant good posture, regular back strengthening exercise, eat a healthy diet to prevent osteoporosis, no smoking, and limit alcohol consumption, maintain a desired healthy body weight, you will help to have a healthy spine without back pain.

常見的腸胃小毛病

A. 胃酸倒流

頻頻倒流=GERD=Gastro-esophagealreflux disease

症狀

  1. 胃感覚灼熱或從腹部中間至胸骨之下有燃燒的感覺
  2. 同時在口裏有酸味的感覺, 或有很多口水
  3. 晚上躺下時, 在口內有返胃的感覺

起因

  1. 在懷孕時-通常在前三個月, 女性荷爾蒙的分泌過多而引致排出過多胃酸
  2. 服用某些藥物---例如抗生素
  3. 攝取酒精
  4. 食用某些食物---如辣, 酸或油炸的食物
  5. 有哮喘症狀---呼吸急速或氣喘

治療

  1. 生活的改變

    胃酸倒流多數都是發生在晚上

    • 提高牀頭---使頭部位置高於腹部
    • 提升床頭至少30角度, 可放堅固的楔形枕頭在您的頭及肩膊之下
    • 飯後不可立刻躺平, 讓食物消化三十分鐘以上才可躺下
  2. 躺下時應睡在左邊---  睡在右邊及俯伏地睡, 會使胃酸倒流更惡化
  3. 仔細咀嚼食物---至少要花20秒鐘去咀嚼每一口食物.消化程序從口開始. 若食物在口中没有經過仔細咀嚼會導致消化不良而引致胃酸倒流
  4. 少食多餐---能夠控制食量是處理胃酸倒流的最佳辦法, 建議進食時不可過飽, 因過飽可使胃礦張
  5. 減低高脂肪及澱粉質食物(麵包, 飯, 意大利粉及粉麵類).
  6. 選擇適當的飯後活動---建議飯後等一個半小時才做運動. 若曾有胃酸倒流症狀, 最好不要做彎腰動作
  7. 戒煙及減少攝取酒精---吸煙及飲過量的酒會激發胃排出過量的胃酸.
  8. 保持健康的體重會有幫助, 因為多餘的脂肪會壓着食道下部的括約肌. 
  9. 咀嚼香口膠---嚼香口膠會幫助排出多些口水, 可中和胃酸.睡前可嚼一兩片香口膠會有幫助.  若有輕微的消化不良, 可飲一杯溫暖的清水. 最好在睡前不飲含有咖啡因的飲品, 如咖啡或茶.

     

B. 便秘

日常不須要每天都排便----若一天三次或一星期三次都屬正常, 只要不是長期有大便稀溏或有黑色及大便有血, 便是正常.

嚴重的便秘

  • 感覺有肚子不舒服及/或有肚痛,
  • 一星期沒有大便
  • 曾試用藥房自購的通便藥物, 如瀉藥及金油條, 還未能緩解便秘. 並持續有以上的症狀,便 應去見醫生. 或有嚴重的作悶及嘔吐應要立刻去急症室檢驗是否有更嚴重的病症.

預防便秘

  1. 整日飲多量的流質
  2. 選擇進食高纖維的食物, 如全穀類, 生果及疏菜
  3. 小心服用任何藥物,含有麻醉劑及鴉片劑的藥物, 可引致便秘

便秘的併發症

便秘, 太硬的大便, 及長期需要使勁排便, 可引發痔瘡, 肛門流血會導致有貧血, 嚴重會導致有肛裂.


Common Gastrointestinal Complaints

A. Reflux

Frequent reflux = GERD = Gastro-esophageal reflux disease

Symptoms

  1. Heartburn-hot or burning feeling arising up from the center of the abdominal area and into the chest under the breast bone.
  2. This may accompany a sour taste in the mouth, or hypersalivation.
  3. Feeling regurgitation of gastric fluid in the mouth especially lying down or at night.

Causes

  1. Pregnancy – usually in the first trimester of pregnancy, with over production of female hormones causing over secretion of stomach acid.
  2. Some medications-like some antibiotics
  3. Alcohol consumptions
  4. Certain food:-spicy, sour or fried food.
  5. Asthma like symptoms:-short of breath or wheezing
  6. Trouble swallowing
  7. Dry cough

Treatment

  1. Life style changes

    Most reflux happens at night:-

    • Raise your head of bed up-position your head higher than your abdomen.
    • Elevate the head of bed at a minimum of 30 degrees by putting a firm foam rubber wedge pillow under your head and shoulder
    • Do not lie down flat immediately after a meal. Give yourself at least 30 minutes to digest your food before lying down.
  2. Sleep on your left side - Sleeping on your right side and lying prone will worsen gastric reflux.
  3. Chew your food well - Chew each bite for at least 20 seconds. Digestion starts in the mouth, if you do not chew your food well you are prone to indigestion leading to reflux.
  4. Eat less but more frequently - Portion control is the best management for acid reflux. Eat until satisfied but not “stuffed”. Over eating causes the stomach to stretch and over production of stomach acid, leading to reflux of acid.
  5. Cut fat and carbohydrates – like fried food, bulky starchy food( bread, rice, pastas and noodles ).
  6. Choose post meal activities wisely – exercise only one and half hour after a meal and try not to do bending exercise if you are experiencing reflux.
  7. Quit smoking and cut back on alcohol consumption. Smoking and drinking excessively stimulates production of stomach acid.
  8. Maintaining a healthy weight will be helpful as extra fat puts pressure on the lower esophageal sphincter.
  9. Chew gums – chewing gums will produce more saliva which helps to neutralize stomach acid. Chew a piece or two before bedtime.Or have slight feeling of indigestion, simply drink a cup of plain warm water. Do not drink caffeinated drinks like coffee or tea before bedtime.

     

B. Constipation

No need to be obsessed with having daily bowel movements – any where between 3 times a day to 3 times a week is normal, as long as it is not always watery or runny or with black stool or blood.

Severe constipation

  • Experiencing abdominal discomfort/pain and/or
  • One week without a bowel movement.
  • Have tried over the counter remedy or laxatives, and glycerin suppository and still constipated. Any above symptoms persist should check in with the doctor, or if severe with nausea and vomiting should go to ER for checkups.

Prevention of constipation

  1. Drink a lot of liquid throughout the day.
  2. Eat a high fiber diet from whole grains, fruit and vegetables.
  3. Beware of any medications that can lead to constipation e.g. narcotics and opiates.

Complications of constipation

Constipation, hard stools, and constant straining can lead to hemorrhoids, causing rectal bleeding and anemia and serious enough will lead to anal fissure.

什麼是支氣管炎及肺炎 ?

支氣管炎-起因

  • 吸入含有化學刺激物體的空氣-氣體如煤氣及其他染污的空氣
  • 曾沒有治好的嚴重感冒可會導致支氣管炎

症狀

  • 咳嗽時有黃綠色的濃痰
  • 由於頻頻咳嗽引致胸骨痛
  • 發燒
  • 週身疲倦

肺炎起因

  • 通常發生在冬天及春天
  • 致感染菌-肺炎球菌
  • 長期使用壓制免疫藥物, 抗血癌藥及腎上腺皮層藥

症狀

  • 發燒(高温度) 華氏104-106 度
  • 發高燒可導致發冷顫
  • 由於劇烈的胸痛而引致不能深呼吸
  • 氣喘
  • 脈膊快速
  • 呼吸緊速
  • 呼吸困難
  • 臉部發紅赤熱
  • 嚴重時,口脣及指甲決氧氣可變紫藍色
  • 不安定
  • 嚴重時-神智不清

治療

  1. 抗生素 -口服或血管注射
  2. 在牀休息, 如氣速便須要在牀坐起來
  3. 有須要時, 要用氧氣,可幫助呼吸通順
  4. 有痛時服止痛藥, 有咳可服止咳藥, 有高體温服降體温藥及舒緩肌肉痛
  5. 飲大量流質可保持有足夠的水份及可鬆化痰液
  6. 營養-提供進食易消化食物

 

What is Bronchitis and Pneumonia?

Bronchitis- causes

  • Inhalation of air and chemical irritants-like gas, or other air contaminants
  • Has severe flu not treated, may lead to bronchitis

Signs and symptoms

  • Cough with purulent sputum
  • Sternal soreness from frequent cough
  • Fever
  • General malaise

Pneumonia-causes

  • Most prevalent during winter and spring
  • Infective agent-Pneumococci
  • Prolonged use of immune-suppressive drugs, anti-leukemic drugs and corticosteroids

Signs and symptoms

  • Fever( High temperature ) 104 to 106 degrees F
  • High fever can lead to chills and rigor
  • Sharp pain in the chest prevent one from taking deep breath
  • Short of breath
  • Rapid pulse
  • Rapid respiration
  • Dyspnoea
  • Flushed face
  • Severe stage,cyanotic lips and nails
  • Restless
  • Delirium(severe stage)

Treatment

  1. Antibiotics –oral or intravenous
  2. Bed rest, if dysnoeic, sit up in bed
  3. Use oxygen when needed to help to breathe better
  4. Analgesics for pain, anti-tussive medication to alleviate cough, anti-pyretics for elevated temperature and to relieve muscular pain
  5. Drink plenty of fluid to maintain hydration and to loosen sputum
  6. Nutrition-soft and easy digestible food is recommended

什麼是糖尿病前期?

糖尿病前期是當一個人不能正常消化體內的糖份或稱空腹血糖異常

-在美國有九千萬二十歲以上的人都有糖尿病前期, 有空腹血糖高過正常, 但又未診斷為糖尿病。

-及早診斷糖尿病前期是很重要的,因為可防止有二型糖尿病的發生及防止因它所引致的併發症, 如心臟,血管, 腦, 眼及腎的疾病發生。

糖尿病前期的症狀

  1. 大多數人都沒有症狀
  2. 有時會有早期二型糖尿病的症狀, 例如有異常的口渴, 頻頻小便, 有些疲倦及眼朦。

誰應該要檢驗

  1.  四十五歲以上的人仕或長者。
  2. 過重的人, 有體重指數(BMI-25及以上)或亞州人BMI高過23, 而又有以下的風險:-
  • 不活躍
  • 直系親屬(如父母親及兄弟姊妹) 有糖尿病歷史
  • 其他有高風險族群:- 亞州人, 非裔美國人, 亞太人, 拉美裔人, 及美州印弟安人
  • 曾有妊娠糖尿病及產下超過九磅的嬰兒
  • 有多囊性卵巢疾病
  • 有高膽固醇, 高三甘油脂及低的高飽和脂肪的膽固醇
  • 有兩次或多次不正常的高血糖檢驗報告
  • 有心臟病的歷史

 

怎樣診斷糖尿病前期?

您的家庭醫生會檢驗以下其中不同的血檢驗:-

1. 空肚驗血糖 -正常-70-99mg/dl(3.88-5.5mmol/L)

    糖尿病前期-100mg-125mg/dl (5.55-6.94mmol/L)

    糖尿病-空肚血糖度數-兩次或多次保持高過正常度數

2. 糖耐量異常度數-OGTT-(Oral Glucose Tolerance Test)

     正常-低過140mg/dl (7.8mmol/L)

     糖尿病前期-由140-199mg/dl (7.8 to 11.1mmol/L)

     糖尿病-高過200mg/L (11.11mmol/L)

3.  糖化血紅素(Hgb A1c) 

     正常5.7%或低過5.7%

     糖尿病前期-5.7%至6.4%

     糖尿病-6.5% 或高過6.5%

若您有兩次或以上的空肚血糖檢驗是在126mg/dl  您的醫生便會通知您患有糖尿病。

為什麼察覺及治療糖尿病前期是那麽重要呢?

-注意糖尿病前期的象徵, 在診斷有糖尿病之前可防止有二型糖尿病及可降低有糖尿病併發症的風險, 如心臓病, 腎及眼疾病, 腸胃病, 神經組織疾病, 及中風。  

研究報告顯示,防止糖尿病的計劃, 可助於防止有糖尿病的發生, 並提供以下的建議:-

  • 健康生活的改變
  • 飲食的改變
  • 體力活動-每天做三十分鐘適度的運動
  • 若您過重-減低4-7%的體重-便可降低糖尿病的機率60%

糖尿病前期的處理

  1. 每星期做五日運動, 每一次三十分鐘, 可分三次10分鐘。步行是最好的運動。在未開始運動計劃之前, 請教您的醫生。
  2. 停止吸煙。若您從未吸過煙, 請不開始。
  3. 聽從您醫生的建議-醫治高血壓, 高膽固朜, 及高三甘油脂。
  4. 若過重-減 5-10%體重, 可有很大的分別。
  5. 健康飲食。若有須要-請教營養師。及學習閱讀食物標籤, 防止進食有高糖份的食物及飲品。
  6. 若您有糖尿病前期的症狀, 請盡快請教您的家庭醫生。
  7. 學習最新的糖尿病及糖尿病前期的知識(閱讀醫學刊物,參加糖尿病會議及與專業人仕傾談)。

What is prediabetes?

Prediabetes is a condition when one has impaired glucose tolerance or 

-Impaired fasting blood glucose – a health condition that has no symptoms.

-About 90 millions people in the U.S. over 20 years old have prediabetes, with fasting blood glucose higher than normal, but are not high enough to be classified as diabetes.

-Early diagnosis of prediabetes is important, and may prevent type 2 diabetes as well as complications, such as heart and blood vessels diseases, eye and kidney diseases.

Symptoms of Prediabetes

  1. Most people have no symptoms.
  2. Early signs of diabetes mellitus e.g. unusual thirst, frequent urination, a little fatigue, and blurred vision.

Who should get tested?

  1. 45 years of age or older.
  2. Overweight people with a BMI ( Body Mass Index ) of 25 or 23 for Asian, and have the following risk factors:-
  • Physically inactive
  • First degree relative with Diabetes, e.g. parents, and siblings.
  • High risk ethnic groups:- Asian, African American, Asian Pacific Islander, Hispanic, and American Indian.
  • History of gestational diabetes or delivered an over 9 pounds baby.
  • Ovarian Polycystic Syndrome.
  • High cholesterol, high triglycerides, or low HDL(good) cholesterol.                
  • Abnormal blood glucose tests in the past or on 2 occasions.
  • History of heart disease.

How is Prediabetes diagnosed?

Your family doctor will perform one of the three different blood tests:-

1. Fasting blood glucose (FBG)-normal – 70-99mg/dl(3.88-5.5mmol/L)

    Prediabetes-100mg-125mg/dl( 5.55-6.94mmol/L)

2. Oral Glucose Tolerance Test (OGTT)-normal-less than 140mg/dl (7.8mmol/L)

     Prediabetes-from 140-199mg/dl (7.8 to 11.1mmol/L)

3. Hemoglobin A1c ( average 3 months BG ) test-

     Normal- 5.6% or less

     Prediabetes – 5.7% to 6.4%

     Diabetes – 6.5% or higher

If Fasting Blood Glucose is 126mg/dl or greater on 2 or more tests then your doctor may notify you that you have diabetes.

Why is it so important to recognize and treat Prediabetes?

-Identify the signs of prediabetes before diabetes is diagnosed can prevent Type 2 diabetes and lower risk of diabetic complications e.g. heart diseases, kidney and eye diseases, stomach and bowel diseases, nerve endings diseases, and stroke.

Research study shows Diabetes Prevention Program (DPP) can prevent diabetes to happen by the following recommendations:-

  • Healthy life changes
  • Diet changes
  • Physical activity-30 minutes of moderate physical activity per day
  • 4-7% of weight loss if you are overweight –can help in 60% reduction in Diabetes Mellitus

Treatment of Prediabetes

  1. Exercise 30 minutes -5 days a week, can be 3 sessions of 10 minutes. Walking is the best exercise-check with your doctor before starting any exercise program.
  2. Stop smoking, and do not start if you have not been smoking.
  3. Treat high blood pressure, high cholesterol and high triglycerides per your doctor’s advices.
  4. Lose weight 5-10%, can make a big difference.
  5. Eat a healthy diet. Consult a dietitian if needed.  And learn to read food labels to avoid high glucose food and drinks.
  6. Consult your primary care physician as soon as possible when you have signs and symptoms of prediabetes.
  7. Educate yourself with updated diabetic and prediabetic knowledge,(reading medical journals, attending diabetic conferences and talking to professionals ).
  8. Be your own advocate. Consult professionals on any diabetic questions.

痛風症 – Gout

什麽是痛風症

痛風症是痛的風濕症。通常在大腳指,拇指,或在身體內其他細小的骨節,如手指或腳指內感覺到。

What is Gout?

Gout is a painful arthritis, commonly felt in a big toe, or thumb, or in any small joints in your body, like the fingers and toes.

 

起因

由於血內的尿酸積聚,稱為高尿酸血。當有痛風症發生時,通常尿酸型成水晶體聚在骨節內,使到它發炎,紅,腫及痛。當您的血內尿酸升高時,您的痛風症發生的機會或會提高。若痛風症不合時醫治,過後,這多次的突發症會更利害,更持久,會可更多次發生,會造成衰弱。有多數人認為痛風症時常有痛的打擊,但痛風症可以在隔有痛時可靜靜的進展,長期會損害骨節。

Causes

By a buildup of uric acid in the blood, known as hyperuricemia.  A gout attack happens when uric acid forms crystals in the joint, causing inflammation, swelling, and pain.  As your blood uric acid level rises, so does the chance for gout and gout attacks.  If gout is not treated, over time, these attacks, or flares, can become severe, last longer, happen more often and one will become more debilitated.

 

急性比慢性痛風症

急性痛風=有痛的痛風症

慢性痛風=慢性的長期發炎及有痛, 可影响多過一個關節。

Acute versus Chronic Gout

Acute Gout=painful gout attack

Chronic Gout=chronic inflammation and pain which can affect more than one joint.

 

誰會有高風險?

-在40-50歲的男仕,痛風症發生多過女人。有發炎的痛風症常發生在男人身上。

-女人通常在收經後,年長才有痛風症。

-很多人有很家族痛風症的歷史。

-有些人有很多種長期性疾病如高血壓,糖尿病,肥胖及腎病,也可能有痛風症。

Who is at risk for gout?

-Men, between 40-50 years old, are more likely to develop gout than women. Gout is the most common form of inflammatory arthritis in men.

-Women develop gout at an older age than men, typically after menopause.

-Many people have family history of gout.

-People with certain health conditions, including high blood pressure, diabetes, obesity, and/or kidney problems.

 

痛風症的症狀

-通常突然發生在晚上,有劇痛。

-受影响的骨節會感覺發熱,紅及腫,有時會輕摸也很痛。

-痛風常發生在大姆指,大腳指及其他關節。

-研究証實,痛風76%發生在大腳指,50%在腳踭及腳,32%在膝頭,

 25%在手指,11%在多過一個部位,及10%在手腕。

Gout- Signs and Symptoms

-Sudden attack of severe pain usually happens at night, without warning.

-Affected joint/joints become hot, red and swollen, at times tender to touch.

-It happens more often in the big toe or the thumb, or any other parts of the body.

-Researches showed that gout occurs 76% in big toe, 50% in ankle or foot, 32% in knees, 25% in fingers, 11% in more than one site and 10% in wrist.

 

痛風症怎樣進展?

第一次經驗有痛風症,常突然發生,沒有警訊,常在半夜有劇痛,維持一個或多過一星期。若血尿酸過高,在兩次痛風期間可損害關節。受害的關節及筋只能有限的活動。

How Gout progresses?

First experience of gout, it comes suddenly, without warning, severe pain in the middle of the night for a week or longer. One may have gout attacks for years, gout can become a chronic disease, which means it may get worse over time if not treated.  

Gout can progress silently, before the first painful attack. If blood uric level is high, joint damage may occur even between attacks.  Deposit of uric acid crystals-called Tophi – may form in the joints.  Long term consequences may include chronic pain that does not go away, damage to joints, and limited movement in the affected joints and tendons.

 

痛風症的四個階段

第一階段----高血尿酸,但沒有症狀,及沒有痛風症。若有痛風症,通常有多過二十年高血尿酸。在第一次痛風突發時,很重要保持血尿酸低過6mg/dl。

應請問您的醫生做第一次血尿酸檢驗,知道您的初步報告。

The 4 stages of gout

Stage 1 – High blood uric acid levels – no symptoms. During the first stage, one may have high uric acid level, but have no symptoms, and don’t actually have gout. Those who have gout may have more than 20 years of high blood level of uric acid level before first gout attack.  It is very important to maintain your blood uric acid level below 6mg/dl.  Ask your doctor for a simple blood test of uric acid to know your baseline level.

 

第二階段----有痛的襲擊痛風症,腳腫及痛,有時不能穿鞋或走幾步路。有時就算很輕簿的床單接觸到痛的關節都感覺到極度痛楚。

State 2 – Gout attack. A gout attack is very painful.  One may not be able to put on shoes or even walk a few steps.  Sometimes even a light bed sheet touching the painful joint can be agonizing.

 

怎樣會觸發痛風的襲擊呢?

體內的尿酸積聚會引起痛風。但有時在沒有特別的原因或可發生。有時酒精,有些藥物,有另外的病或在生活中有特發事情令壓力增加都可觸發有痛風症。

其他觸發痛風的原因包括:-

  • 骨節受傷
  • 手術
  • 吃太多一些食物
  • 暴食
  • 感染                                      

What triggers a gout attack?

A buildup of uric acid in the body causes gout.  It sometimes occurs for no apparent reasons. Sometimes alcohol, certain medicines, another illness or stressful events may trigger gout attacks.

Other known gout triggers include:-

  • Joint Injury
  • Surgery
  • Eating too much certain foods
  • Crash diets
  • Infection

 

第三階段----當突發痛風經正當處理後應與醫生商談您如何可以日後减低痛風突發的風險。很不幸有很多人有突發痛風在一年內又會復發。

Stage 3 – Between Gout Attacks

Once the immediate gout attack is over after proper management, it is time to talk to your doctor about ways to reduce the risk of having attacks in the future.

Unfortunately many who have suffered an acute gout attack will suffer another one within a year.

 

第四階段----長期痛風症-------當血尿酸度數長期高過6mg/dl及重覆有多年痛風突發症,痛風便成長期性。

Stage 4 – Chronic Gout

When blood uric acid level maintain higher than 6mg/dl and repeated gout attacks over years, gout will become chronic.
 

短期及長期性有效治療目標應該注重:-

  • 減低痛風突發
  • 防止未來的痛風症            
  • 降低高血尿酸到健康度數
  • 改變生活習慣,如不飲酒,不吃有些食物及做定期運動。

Short and long term goals of effective gout treatment should aim at:-

  • Reduce the pain of gout attacks
  • Prevent painful future attacks
  • Lower high uric acid to a healthy level
  • Make diet or lifestyle changes, such as refrain from alcohol, avoiding certain food and getting regular exercise.

                                                      

降低血尿酸度數

目標------是保持低過6mg/dl。若您不知道自己的血尿酸度數,可問您的醫生。

高過6mg/dl的血尿酸=高風險未來有痛風。

低過6mg/dl的血尿酸=低風險未來有痛風。

Lowering Uric Acid Level

Goal – is to maintain less than 6mg/dl.  Ask your doctor for a blood test of uric acid if you don’t know your uric acid level. 

Higher than 6mg/dl uric acid level=Higher risk of future attacks. 

Lower than 6mg/dl uric acid level=lower risk of future attacks.

 

治療

-藥物可治療痛風----藥物可在您的身體降低及排出積聚過多的尿酸,幫助减低痛風突發的風險。您需要長期保持正常的度數。

-高嘌呤的食物的作用----您的血尿酸是從您的身體排出。嘌呤是一種物質在您的身體內,也可以在很多食物內。三份二的尿酸是從您的身體內自然排出,而三份一是從飲食內攝取。高嘌呤的食物可觸發痛風突發,但不是痛風的起因。

-飲大量沒有酒精的飲料如清水會有幫助。

-健康生活的改變------保持健康體重、定期做運動、若您知道進食有些食

 物或飲料使到您有痛風突發,便不要選擇那種食物及飲料。

有些人仕就算跟進最嚴格低嘌呤的飲食, 通常都不能降低他們的尿酸度數多過1mg/dl.

Treatment

1.  Medication for the treatment of gout.  

Medication helps to reduce the production of excess uric acid in your body. To reduce your risk of gout flares, you need to keep your blood uric acid level down and keep it below 6mg/dl at all times.

2.  The role of high purine foods

The uric acid in your blood comes from purines, substances that are produced by your body and also found in many food. Most of your uric acid (2/3) is produced naturally by your body, while the rest (1/3) comes from your diet. High purine diet can trigger a gout attack but it is not the cause.

3.  Drinking plenty of non-alcoholic beverages especially water will help.

     -Maintain a healthy weight

     -Getting regular exercises

     -If you notice after eating certain food and drinking certain beverages you have a gout flare, avoid those choices.

   -Even some people follow the strictest low-purine diet, generally don’t reduce their uric acid levels by not much   more than 1mg/dl.

Healthy tips

a) Drink plenty of liquids, like water.  Fluids like water help to remove uric acid from your body.

b) Add low-fat dairy products to your diet.  Eating more of the low-fat dairy product is associated with a lower risk of gout.

c) Avoiding or limiting high purine food may help to reduce the risk to trigger a gout attack if you already have a high uric acid level.

-Vegetables-spinach, asparagus, cauliflower and mushrooms.

-Others-Oatmeal, peas, dried beans and lentils.

健康飲食  Nutrition for Better Health

健康的飲食能降低很多慢性疾病的風險,例如心臟病、癌症、糖尿病、及其他慢性疾病,以及同時讓我們控制體重來維持健康,或讓我們感覺舒服點。

健康飲食其實並不需要很、或無味道、或不吸引、或者很複雜。健康飲食可幫助控制體重,尤其從限制食物的份量及多做運動入手功效更為顯著。健康飲食有多種選擇,讓你嘗試多種不同的食物,如蔬菜、全麥、或是你不常吃的水果。

均衡的飲食是包括吸取豐富的營養及纖維。有時候吃一個漢堡包或享受一小杯雪糕都無大礙,但要記住限制次數及控制份量。

Many of us are trying to eat better in order to lower the risk of many diseases e.g. heart disease, cancer, diabetes, and other chronic diseases, as well as controlling our weight to become healthier or simply just want to feel good.

Healthy eating does not have to be too restrictive, bland, unappealing and confusing.  A healthy diet can help with weight control, especially effective if you limit portion sizes and start to exercise more.  Healthy eating is to allow you to have many choices of foods, especially vegetables, whole grains, and fruit that you don’t normally eat.

A balanced healthy diet is rich in nutrients and fiber.  It is fine to occasionally enjoy a hamburger or a cone of ice cream, which is acceptable as long as we limit the frequency of these foods and exercise portion control.

 


十一種健康飲食指引   11 Keys to Healthier Diet

1.  保持進餐份量適中。例如在外面吃飯,選擇頭盤或前菜當主餐或與別人分享同吃一個主餐。

1.  Keep portions moderate.  When dining out, choose an appetizer instead of an entrée or split an order with someone.

 

2.  吃多種不同類的食物。所有的食物都有不同的營養價值及保健益處。攝取多種不同的食物的營養可幫助你的身體抵抗很多疾病。

2.  Eat a variety of food.  Not all nutrients in food have the same health benefits.  Eating a wide variety of food helps to ensure that you will get all the disease fighting potentials that food offers.

 

3.  注重高纖維食物,例如蔬菜,水果,豆類及全麥類。這些好的碳水化合物其實都有豐富的營養,令你感覺飽滿而且又低卡路里。高纖維的食物可幫助減慢碳水化合物的吸收,使你的血糖不會突然飆升, 尤其對糖尿病患者有幫助。

3.  Focus on high fiber food like vegetables, fruits, legumes, and whole grains.  These good carbohydrates are nutritious, filling and relatively low in calories.  Fiber slows the absorption of carbohydrates, thus not getting your blood sugar spikes in your body and is beneficial, especially helpful for any diabetics.

 

4.  吃大量的新鮮蔬果。每日至少吃五份的水果或蔬菜。通常半杯左右的蔬菜或水果等於一份。盡量吃多種顏色的蔬果--紅、橙、黄、綠、青、藍、紫。蔬果除了含有多種纖維外,還含多種微量營養素(micronutrient)及植物化學物質(phytochemicals) ,能抗癌及抵抗其他疾病。豆類也屬於疏菜類,卡路里含量比蔬菜多,同時使你更易感覺飽滿。多吃一整個水果面盡量少喝果汁,因為水果的大量纖維可幫助防止結腸癌。

4.  Eat lots of fresh produce.  Eat at least 5 servings of fruit and vegetables per day.  Normally ½ cup consider as one serving.  Eat produce in multiple colors – red, orange, yellow, green, and blue/purple.  Besides having lots of fibers in produce, they contain many micro-nutrients and phytochemicals, it may help to protect against cancer and other diseases. Legumes are considered as vegetables, it has more calories than most vegetables and it makes you feel fuller.  Eat more whole fruits than drinking fruit juices, as it has more fibers-it helps in preventing colon cancers.

 

5.  多吃五穀類食物。應多吃全穀類,如全麥及燕麥。一份的份量不是很多,例如一片全麥麵包,半杯煮熟麥片或糙米。

5.  Eat more whole grains.  Half of your grains should be whole grains, e.g. whole wheat, and oats.  Servings are small, e.g. 1 slice of whole wheat bread, half a cup of cooked oatmeal or brown rice.

 

6.  限制進食精製的碳水化合物,例如白麵包、白飯、普通粉麵、含糖零食、糕點、餡餅(pie)、蛋糕及含糖飲料。這些食物沒多太多營養及低纖維,而且含糖量高的食物都是高脂肪及高卡路里,容易引致肥胖及增重。

6.  Limit intake of refined carbohydrates, e.g. White bread, white rice, regular pasta, sugary snacks, sugary pastries, pies, cakes, candies and sugary drinks.  These foods have very little fiber and not many nutrients.  Many sugary foods are high in fat and calorie dense which will contribute to weight gain.

 

7.  注重好的脂肪(不飽和脂肪)。不飽和脂肪通常可透過堅果、魚、菜油及橄欖油中攝取。用橄欖油及菜籽油來代替牛油。吃含富脂肪的魚,可幫助減低心臟病的風險、軟化血管、及防止中風。魚含大量奧米加三(Omega 3) 的不飽和脂肪。無魚鱗的魚,如三文魚、吞拿魚、鯡魚、沙甸魚都有很好的不飽和脂肪。但需要注意的是,無魚鱗的深海魚含水銀量可能會比人工飼養的魚較多,而人工飼養的魚則可能有含有化學物、抗生素或類固醇。每星期吃兩至三次魚應該可獲得需要的健康益處。

7.  Emphasize on “good” (unsaturated) fats.  Normally found in nuts, fish, avocados, and vegetables and olive oil.   Substitute olive or canola oil for butter.  Fatty fish helps to reduce the risk of heart disease, soften blood vessels to prevent strokes.  Fish also has omega-3 polyunsaturated fats.  Scaleless fish like salmon, tuna, herring and sardines have good un-saturated fats. Be alert, scaleless deep sea wild fishes may have more mercury than farm raised ones, but farm raised fishes may have more other chemicals, antibiotics or steroids.  Therefore eat fishes 2-3 times per week to get health benefits.

 

8.  限制進食肉類食物。動物的肉內含有很高飽和脂肪。飽和脂肪可使你的低密度脂蛋白膽固醇(LDL)升高。正常膽固醇(150-200mg/dl);正常三酯甘油酸(低於150mg/dl);正常高密度脂蛋白(HDL)(40-59mg/dl);低密度脂蛋白(LDL) 膽固醇 (<130mg/dl)。HDL數字越高越好,而LDL數字越低越好,但當然過低或過高都不理想。HDL 能幫助清理血管內的斑塊 (plague),而LDL會形成斑塊,讓血管收窄及全部阻塞,長期會導致突發心臟病及中風。若你的驗血報告內這些數字不好,你的家庭醫生或許會要求你降低這些數字來減輕罹患心臟病、高血壓及中風的風險,預防其他多種疾病。建議進食有限量的飽和脂肪,可選擇瘦肉、脫皮雞鴨、脫脂或低脂奶類食物。

8.  Limit intake of saturated fats-Found in animal meat. Saturated fats will get LDL “bad” cholesterol level up.  Normal Cholesterol level (150-200mg/dl); Normal Triglyceride <150mg/dl; HDL – 40-59mg/dl (The higher the better); LDL - <130mg/dl (The lower the better).  HDL helps to clean up the plague inside the blood vessels and LDL will form plague and eventually will get blood vessels narrowed and will be completely blocked in the long run, causing heart attacks and strokes.  If one has heart disease, hypertension, or at risk for stroke, your PMD may want to get these numbers lower for better diseases prevention.  Therefore it is advisable to limit your intake of saturated fats – choose lean meats, skinless poultry, nonfat or low fat dairy products.

 

9.  盡量避免進食反式脂肪食物。人工製造的食品大部份都使用氫化菜油或萬年油,像是商業製的焙烘食品,零食以及快餐店食物 (如炸薯條)。反式脂肪使低密度脂蛋白LDL升高以及降低高密度脂蛋白HDL,提高罹患心臟病及中風的風險。

9.  Avoid or minimize trans-fats – Many processed foods contain partially Hydrogenated vegetable oils which are found in commercially baked goods, snack foods and fast foods (such as French Fries). Trans fats raise LDL (bad cholesterols) and reduce HDL (good cholesterols), increasing the risk of heart disease and stroke.

 

10.  限制進食高膽固醇食物,例如肉類、海鮮及蛋。美國糖尿學會建議,健康人仕每日可攝取300毫克膽固醇。一隻雞蛋大約有237毫克膽固醇。若有心臟病、糖尿病、或中風人仕則要限制一日不攝取多於200毫克。

10.  Limit the intake of dietary cholesterol – mostly found in animal foods, meats and eggs.  ADA recommends healthy individual to consume 300mg per day.  One egg contains approximately 237mg.  For those with heart diseases, diabetes and strokes, limit to 200 mg per day.

 

11.  減少吃鈉 / 鹽 (sodium) 及多吃鉀質(Potassium)豐富的食物。過多的鈉 / 鹽會提高血壓。五十歲以上人仕、非裔美國人、糖尿病 / 高血壓 / 心臟病患者、及慢性腎病患者應依從醫生吩咐限制鈉 / 鹽的攝取量,即每日1500毫克 (大慨是三份之二茶匙的鹽)。健康人仕一日應攝取少於2300毫克的鈉 / 鹽。多攝取含鉀質的食物可降低血壓。鉀質豐富的食物包括柑橘類水果、香蕉、蕃薯以及乳酪 (yogurt)。

11.  Cut down on sodium and eat more potassium rich foods. Excessive sodium intake raises blood pressure.  People over 50 years old, African Americans, people with diabetes, hypertension, and chronic kidney diseases should limit sodium to 1500 mg/day (approximately 2/3 of a teaspoon of salt).  Healthy people should consume less than 2300mg of sodium a day.  Consume more potassium can help lower your blood pressure.  Potassium rich foods include citrus fruits, bananas, potatoes, beans and yogurt.

  • 與有感染人及有菌物件接觸Contact with infected person or objects
  • 缺乏睡眠Lack of sleep
  • 傷風症狀Cold – Signs and Symptoms
  • 流鼻水Nasal discharge
  • 鼻塞Nasal Obstruction
  • 口嚨痛Sore throat
  • 打噴嚏Sneezing
  • 疲倦Malaise
  • 低燒 -華氏99-100 度Low grade fever 99-100 degrees F
  • 感覺寒冷Feeling chills
  • 咳嗽有或沒有痰 - 痰可有清色或淺黄色Cough with or without sputum-sputum is clear to pale yellowish
  • 聲嘶/口嚨發炎Hoarseness of voice/laryngitis
  • 症狀或輕微 (帶有輕微症狀)Symptoms may be mild

 

 

 

流行性感冒症狀 Influenza(Flu) – Signs and Symptoms

  • 發燒高過華氏101度Fever – high – over 101degree F
  • 頭痛Headache

  • 咳有綠色痰Cough with greenish sputum
  • 口嚨痛Sore throat
  • 眼或耳痛Eye or ear soreness
  • 心腔緊痛由於頻頻咳Chest tightness due to frequent cough
  • Symptoms maybe more severe症狀嚴重一點

 

傷風及感冒治療Treatment and management for cold and flu

  • 休息-睡眠或少活動Rest – sleep or low activity
  • 足夠流質-飲大量流質如熱湯, 菓汁及開水Enough fluid-drink plenty of fluid-hot soup, juices and water
  • 切勿飲奶/進食奶類食物直至無咳及無痰Quit milk/milk product until no cough or no sputum
  • 有鼻塞買藥房的噴鼻劑OTC Nasal spray or nasal decongestant for blocked nose
  • 聲嘶及喉嚨發炎-用温鹽水漱口Hoarseness of voice and laryngitis-salt with warm water mouth gargle frequently
  • 頭痛, 骨及肌肉痛, 服止痛藥Headache, bone and muscle ache- take analgesics
  • 有感冒及高燒要去見您的醫生-或需要消炎藥Has flu with high fever-go to see your doctor-may need antibiotic
  • 食大量生菓(生菓有大量維他命Eat lots of fruit (fruit has lots of vitamins )

 

預防法Prevention

  • 做好個人衛生-頻頻洗手, 當有咳嗽或打噴嚏時應蓋着口鼻Good personal hygiene-frequent hand washing, and cover your nose and mouth when coughing and sneezing
  • 每年打感冒針Annual flu shot
  • 足夠睡眠Plenty of sleep
  • 室外灑太暘及活動Outdoor sunshine and activity
  • 平衡飲食, 吃大量生菓/疏菜Balanced diet(lots of fruit and vegetables)
  • 減低壓力Reduce stress
  • 學會放鬆治療Relaxation therapy

  • 緊急治療 - 立刻電召911
  • 保持冷靜及保持病人夠暖,若發覺病人跌倒在地上請千萬不可移動他,直至到救護人員抵達現場。 
  • 抵達急症室後,經醫生診斷過後及照完腦電腦圖証實是中風,而腦內有血塊,緊急治療是在中風後四小時內注射血管針藥 – IV tPA,或其他治療是注射或口服薄血藥。這是其中之一種中風情況,還有其他種類,如爆血管、腦血管狹窄、及腦腫瘤(良性或惡性) ,各有不同的處理,有內科或外科治療。
  • 中風後,病人需要入院,會被送到急性中風治療病房。
  • 在急性中風治療期間,病人除了有醫生的醫療及護士的護理外,還有物理治療師、職業治療師、言語治療師及營養師提供中風後的治療。
  • 物理治療師會指導中風受損的手腳做運動,職業治療師會指導做日常生活的需要,例如梳洗、沐浴、更衣、及要用手操作的日常活動,言語治療師會指導言語、講說話及吞嚥,營養師會指導言語治療師所提供適當的營養餐。
  • 當中風情況穩定後,出院計劃護士會指導出院計劃,或需要回家有家居護理,或需要去康復治療中心接受康復治療。
  • 經過數星期的康復治療,病人或會回家繼續做運動,日常生活需要及適合營養餐的指導,若病人家屬不能在家照顧病人,他/她便要去護理院。
  • 有些病人在中風後會完全康復,關鍵在中風的嚴重性,他/她或會繼續他/她的以往日常生活。
  • 中風後,最重要的治療處理是立刻尋求以上提供的治療方法。

Treatment of Stroke

  • Emergency treatment – to call 911 immediately
  • Keep patient calm and warm and do not move patient if he/she fell on floor until paramedic staff arrives.
  • After arrival to emergency room and being seen by doctors and had the brain CT scan to confirm patient had a stroke, with blood clots in the brain, emergency treatment of IV tPA will be given the patient within four hours of the onset of the stoke, or the other treatment is to give anticoagulant, either intravenous (IV), injection or oral medication to anti-coagulate the blood.  This is one kind of strokes, there are other kinds, e.g. ruptured brain blood vessels, narrowing of brain blood vessels, and brain tumor (benign or cancerous ). All have different kinds of treatment, either medical or surgical treatment.
  • After a stroke, patient will be admitted to a specialized unit for acute stroke care.
  • During the acute care period, Physical Therapist (PT), Occupational Therapist (OT), Speech Pathologist (SP), and Dietitian will all be giving treatment to patient, besides doctors giving medical care and nurses giving nursing care.
  • Physical Therapist will instruct about exercising of affected extremities, Occupational Therapist ill instruct on activities of daily living (ADL) e.g. washing, bathing, feeding, dressing, and using f hands and arms to do daily activities, Speech Pathologist will instruct on language, talking and wallowing, and dietitian will instruct on the appropriate diet recommended by Speech Pathologist.
  • When stroke condition is stable, the discharge planning nurse will instruct on discharge planning, either to go home with home care or go to acute rehabilitation center for acute rehabilitation.  
  • After several weeks of rehabilitation, patient either goes home and continues with instructions on exercises, ADL activities, and appropriate diet, or go to nursing home if family is unable to care for patient at home.  
  • Some patients may have complete recovery after a stroke. It depends on the severity of the stroke condition, and he/she may continue with his/her usual daily life.  
  • The most important treatment after a stroke is to seek treatment immediately as above.

上神學課堂時,教授問:主耶穌是百份之幾是天主?百份之幾是人?這是一個看似簡單的問題 ,但大多數教友會答錯。

我們閱讀聖經,研究主耶穌所講的道理,要從天主性同人性兩個角度方面去分析。 例如:在加納婚宴,耶穌的母親向衪說:「他們沒有酒了。」 耶穌回答說:「女人,這於我和妳有什麼關係?我的時刻尚未來到。」

負面情緒泛濫時......如何處理?

當感覺焦慮、恐慌、或負面情緒泛濫時,嘗試暫停,慢呼吸,默禱,回到真實的現在,與主相遇:

1️⃣ 盡可能舒適地坐著。

2️⃣ 將注意力放到自己每一下呼吸,感受吸入、呼出的空氣。

3️⃣ 如有其他思緒浮現,輕輕將注意力回到每一下呼吸。

4️⃣ 選一句短誦呼求天主,如「耶穌」、「天父」、「阿爸,父啊」、「耶穌,我信賴你」,反覆默禱;讓祈禱文於內心迴盪。

5️⃣ 以天主經結束(建議每節約20分鐘)

🙏🏻「上主,求你速來救我:作我避難的磐石,獲救的城堡。」(詠31:3)

有福的人

- 徐明德神父

 

 

記得我在中學的時候,曾經參加教會暑假的學生營隊,有許大哥哥和大姐姐義務地在營隊中服務。幾天的營隊下來,他們的服務熱忱讓我受到感動。營隊結束之前,我向帶領我這一小隊的大哥哥表達感謝,這位大哥哥很有智慧地告訴我:「如果你覺得在這營隊中感受到我們服務人員的愛和照顧的話,也把這份愛傳遞下去,分享給你周圍的人。」他所說的話讓我印象深刻,也讓我領悟到接受愛之後,可以將感恩之情,以實際的行動,將愛傳遞出去,成為一個愛的傳播者。透過這位大哥哥的愛的行動與智慧,使我體會到真愛的無私,我想這是一次我與天主相遇的經驗。

聖週 Holy Week


 

主內的弟兄姊妹們,大家好,

在復活節前的這一星期是聖週 Holy Week,提醒大家可到你們家附近的教堂參與聖週禮儀。如要尋找附近的教堂,可以使用網站 https://masstimes.org/ 並參考該教堂的網站的聖週時間表。請注意,屋崙聖良堂於聖週四、五、六是沒有廣東話禮儀的。在復活節主日(4月1日)當日,星期日下午12時30分,聖良堂會舉行廣東話彌撒。彌撒後會於教堂對面的Father Lacey’s Room舉行復活節聚餐,歡迎各位帶同親朋好友前來參加。

 

聖週禮儀

星期四 – 主的晚餐

星期五 – 救主受難紀念

星期六 – 至聖之夜逾越節守夜禮

星期日 – 耶穌復活主日感恩祭 (聖良堂12時30分廣東話彌撒)

愛天主、愛人

「愛天主在萬有之上,愛人如己。」天主教徒應該知道,這是兩條最大的誡命。生活在這個世代,「愛」這個字時常被濫用。很多球迷說:「我愛我的球隊。」,但當球隊演出失水準,輸了給弱隊時,球迷就會喝倒采,甚至亂丟垃圾來洩憤。很多夫婦在結婚前,表達非常之愛對方,可以奉獻自己一生。但當婚姻生活遇上風浪,就忘記了愛的誓言,認為對方是負累,一於斬纜,各自飛。

【昔日文章】~十四年的回憶~

 

 

 

          一個團體的建立有賴多人的合作、貢獻、苦幹、犧牲和恆心。想當年屋崙教區的粵語華人牧民中心,始自於教區要理中心的一台將臨期首主日的彌撒。當日參加彌撒的人約有20人。但從此後,屋崙教區每主日都有中文彌撒了。那時候華人教友團的中堅分子是十數名在加州柏克萊大學及其他大學就讀的留學生。我們在每次彌撒後都有活動:聖經研究、 歌唱、遊戲、座談會或美食分享。當時美麗湖邊的露德聖母小堂是我們這班教友每主日舉行感恩祭的地方,而露德聖母禮堂也就成了我們經常聚會、計劃和切磋的主要場所。後來咖賴愛德華司鐸調任爲聖良堂主任,批准了我們在聖良堂舉行感恩聖祭,於是我們便由美麗湖移到丕蒙街和烈治韋街的聖良天主堂了......

         在聖堂內領導一個合唱團唱聖歌的人就叫詩歌班指揮。他的任務相當重要。除了指揮一個合唱團外還負責選曲、訓練和在感恩禮儀中指揮演出。蛇無頭不行。合唱團沒有指揮也會散亂,手足無措,拍子容易出錯。指揮在一個合唱團裏面居重要地位,對要唱的歌,有甄選權。要有指揮的資格他必需比其他團員更有音樂水準,對發音和歌詞有更深了解......

十五年回顧

「十五年可以說是一段很長的時間。但當它靜靜的溜走後,又會覺得十五年內發生的事情好像只是剛剛過去,因為記憶猶新。

十五年前的十二月六日,那時候主教大堂,Christ the Light還未建築完成,我們十八位男仕在San Leandro的St. Felicitas Catholic Church由主教 Bishop Allen Vigneron 覆手接受執事聖職。我們非常之感恩及興奮,因為經過五年的培訓,積聚所學的,終於可以學以致用。

This year's Thanksgiving was a memorable one.  The Sunday right after Thanksgiving was Feast of Christ the King, and we had five small children / babies baptized during our Cantonese mass on that day!  Most of them are the 3rd generation of our church, meaning they are the grandchildren of our parishioners.  It was very amazing as I witnessed how faith has been passed down from generation to generation.  It was truly a day full of blessings and we thank God for their new lives.

感謝天主對我們的看顧,賜我們很好的神師,在過去40年服務灣區的華人教友。現在特設此專頁紀念我們已故的神師吳天福神父,並與大家分享各團體及教友和吳天福神父多年來的點滴。請您與我們一起為吳天福神父祈禱,也為我們牧民中心四個華人團體祈禱!

專頁內收集了感恩祈禱會的相片及短片,也收錄了屋崙榮休主教的悼文,以及各團體代表教友的分享。

We thank God for looking after us and giving us a wonderful pastor for the past 30 years, serving the Chinese Catholics in the Bay Area.  We have dedicated a special page in memory of our beloved pastor Fr. Thomas Ng.  Please pray with us for our Fr. Ng, and also pray for our four Chinese communities under the Chinese Pastoral and Cultural Center (under the Diocese of Oakland)!

「講錢失感情」。不管是父母、子女、夫婦,一講到錢,就會有麻煩。由爭遺產的案件,可以看到錢的殺傷力,而親情就沒有價值。事實上,留遺產給人,不是聰明的選擇。那些繼承財產的人,就是不勞而獲的人。他們不會說一聲「多謝」,因為留遺產的已經死了,都聽不到。

如果不希望看見某位親屬或朋友,很簡單,不需要打、不需要鬧,只是問他:「你搵幾多錢?你有幾多身家?」擔保他不會再來見你。不相信?

在馬爾谷福音,主耶穌一提到金錢,那個富有青年就「一聲唔出」,走了,相信以後他不想再見主耶穌。(谷10:21-22)

The Goodness of Prayers

I first learned to pray when I started Kindergarten in an Italian Convent Catholic School in Hong Kong. The school principal started each school day on the microphone with prayers. I prayed as I sang along with my classmates.

Prayers did not mean anything to me at that time. As years went by, I learned that prayers are my conversation with God. I prayed fervently when exams came, like any students in school. Sometimes God heard me, but sometimes He did not. I just wondered, was God too busy to listen to my prayers?

十月份聖人 Saints in October

 

十月一日

聖德蘭.里修(小德肋撒)

(貞女、聖師、傳教區主要主保)(慶日)

聖女於一八七三年在法國出生。年少時即入里修的嘉爾默羅聖衣會修道,專務福音中謙遜、誠樸,以及依恃天主的美德;並以身作則,特別教誨初學的修女。她毋時不忘傳教救靈,並願以祈禱和隱修生活,奉獻生命,為拯救人靈,傳揚聖道。一八九七年九月三十日安逝。教會於一九二五年宣布她為聖人,並立她為傳教區的主保。一九九七年,教宗若望保祿二世立她為教會聖師。

 

十月十五日

聖德蘭.亞維拉(大德肋撒)

(貞女、聖師)(紀念)

聖女在一五一五年出生於西班牙之亞維拉。及長,入聖衣會潛修,經歷內心的掙扎,蒙受主恩,聖德日進。聖女竭力重整會規,受苦良多,卻能百折不撓,終於成功。她也曾協助聖十字架若望,重整聖衣男修會。聖女曾撰寫許多靈修生活的書籍,道出個人的神修經驗,教導人與主契合。一五八二年安逝於亞爾巴。在一九七○年,教宗保祿六世立她為聖師。

Eulogy

Reverend Thomas Ng

The ministry of Father Thomas Ng in the Diocese of Oakland I would describe as providential. The hand of God was in it. Without him one would read a different history of both diocese and of our Chinese community.

Our first bishop Floyd Begin coming from Ohio quickly recognized the diversity that is Northern California, the Bay Area and particularly the Diocese of Oakland. He established vicariates to recognize the major communities. One of those was Chinese, noted much by the presence of Father Matthias Lu. Exiled from his own country and urged by the apostolic delegate in the United States to be situated at St. Mary’s College in Moraga, Father Matthias’ scholarly work of translating Thomas Aquinas was a time consuming operation. Father Lu also was aging.

光榮十字聖架

「誰若願意跟隨我,該棄絕自己,背著自己的十字架來跟隨我。」(瑪:16:24)這句福音對公元313年前的基督徒是每天都要做的。因為那時候跟隨基督的信徒,每天過着心驚膽顫的生活,好像背著一個被迫害的十字架。他們的敘會是秘密的,如果給羅馬政府捉住,極有可能被送到鬥獸場餵野獸。雖然如此,但他們仍然為了信基督而不害怕死亡。 早幾拾年,在中國大陸亦有主教、神父、修女、平信徒每日背著基督的十字架在監獄或勞改營生活。他們受的苦難與公元313年前在羅馬帝國統治下生活的基督徒一樣。

For Chinese Catholics, perhaps no evangelist figures as prominently as Servant of God Matteo Ricci, S.J. (利瑪竇), who was invited into Imperial China in the late 1500s and founded the historic Cathedral of the Immaculate Conception, the oldest church in Beijing. Ricci’s model of accommodating for cultural differences without distorting the Gospel message increased how native Chinese viewed the word of God.

Centuries later, when Chinese Christians emigrated from Hong Kong, Macau, and Taiwan to foreign countries, they needed evangelizers and spiritual directors who could connect with them.

吳天福神父 Fr. Thomas Ng

我們親愛的吳天福神父於二零一八年七月三十一日,在美國三藩市去世,享年八十六歲。

守夜禮及殯葬彌撒將於屋崙聖良堂舉行 (Google地圖) 。

守夜禮:  8月20日(星期一),下午5時瞻仰遺容,守靈禮7時開始。

殯葬彌撒:  8月21日(星期二)早上10時正,由屋崙主教Bishop Barber主祭。彌撒後隨即安葬於Hayward市的Holy Sepulchre Cemetery (Google地圖)。

With our deepest sorrow, we would like to inform you that our beloved Reverend Fr. Thomas Ng, 86, passed away on July 31, 2018.

His vigil and funeral mass will be held at Oakland St. Leo the Great Church (Google Map). 

Vigil: Monday, August 20.  Public viewing at 5pm, Vigil Service begins at 7 pm

Funeral Mass: Tuesday, August 21, at 10 am.  Bishop Barber from the Diocese of Oakland will be the mass celebrant.  Burial will be held at Holy Sepulchre Cemetery in Hayward after mass (Google Map).

As Christians, we are taught to evangelize, that is, to share the Good News with other people we encounter.

We’re told it’s our responsibility.

For me, it’s not something I have an easy time doing. In an increasingly secular society, where “Jesus Freaks” are not admired as much as they are ridiculed, and where we can get into legal trouble if we don’t properly separate Church and State, I don’t want to risk alienating others by sharing with them my religious beliefs. But I – shamefully – know that the more honest statement is: I don’t want to risk alienating myself with others.

2018年度的加州天主教華人生活營 (CACCLC)將會在8/31至9/3的勞工節長週末在Three Rivers, CA 舉行。今年的主題是「以思想,心靈和雙手實践信仰」。主講神師是Fr. James L. Heft。歡迎所有18歲以上的青年朋友與我們一起共度愉快的週末,加深我們的信仰生活,與主相遇。報名截止8月24日:學生/非就業人士收費為$180,就業人士收費 為$230。詳情請見CACCLC官網cacclc.org.
